AUTHORIZATION AND RELEASE OF LIABILITY:
I, the parent or guardian of the above-named child, authorize the participation of my child in Midget Football, I recognize as with all contact sports there are inherent risks involved. In consideration for my child`s participation in this program, and on behalf of my child and me as a parent or guardian, I hereby release, discharge, hold harmless and indemnify, and covenant not to sue, the Battle River Shock Football Club and all the societies directors, volunteers, coaches and other game day workers. CONSENT TO MEDICAL TREATMENT:
In the event my child is injured or needs medical attention, I hereby authorize the Battle River Shock Football Club’s authorized staff to seek medical attention as they see fit for the treatment of my child. INTERVIEW / PHOTOGRAPH / AUDIO / VIDEO CONSENT
At any given time Battle River Shock Football Club members or members of the local Media may show up with cameras to record the activity in the Capital District Football Midget Football League and / or Battle River Shock Football Events for promotional purposes. By consenting underneath the parent / guardian of the above named child acknowledges that this activity may take place and the parent / guardian consent to the use of their child’s image and / or name as recorded for these purposes by Battle River Shock Football Club or members of the local Media/Press. PARENT CODE OF CONDUCT
A parent’s expectations and attitudes have a significant bearing on a child’s attitude towards other players, officials, managers, coaches and other parents. As a parent, I will:
1. exhibit good sportsmanship at all times, never condone violations of the rules of the game or exhibit behavior contrary to the spirit of the rules of the game.
2. place the well-being and safety of each player above all considerations.
3. not make abusive remarks to any official, player, coaches, volunteer or league administrative personnel.
4. Respect and show appreciation for the trained volunteers, coaches, officials, managers and trainers and remember that children learn best by example; and,

REGISTRATION PROCEDURE & CHECK LIST:

Online Registration is completed fully (all fields must be filled in)
Medical Form printed and completed fully. A new form must be filled out even if they played Shock in 2023

Copy of Report Card (grades can be blacked out) – We just need report cards to prove what grade the player is in. If the school uses PowerSchool, we can accept the front page saying what grade they are in

Registration Fee – Tryout Fee is $200.00 (The tryout fee includes the combine, 3 tryouts and a t-shirt.) payable by cash on the first day of tryouts or by e-transfer to admin@camroseminorfootball.com.
If paying by E-Transfer please make sure you are writing the players’ name in the memo line of the transfer so we know who it is for.
The regular Season Fee for 2024 is $500.00 (regular season fees include the players’ banquet ticket, their photo, and a practice jersey.)
Equipment Use FEE is $400.00 if Equipment is NOT RETURNED. (This is done on an honor system. If you do not return your equipment, you will be required to pay this)

We must have ALL forms and copies, without them your child is NOT allowed on the field for any practices or games.
